FSUID Web Page was Updated for JES

The new version FSUID was updated in the production today.
The main changes:
- 1. In the Activating FSUID process:

Use CARS_DBA.CARS_UTILS.NEWACCOUNT2 to create new account. (The code will set fsuEduJesStatus to "requested" and add newUserName@fsu.edu into fsuEduFire2Email after this function call.)

Only create employee styled username when somebody has both employee and student roles
- Only ask the password one time in the process
- 2. On the Account List page:

Do not list the UCS account if the email is jesed
- Use the value in attribute "mail" rather than attribute "fsuEduPreferredEmail" as the email of the FSUID account
- The link of "Get Your FSU Email Alias" will go to a new page to check if the current email is upgraded and show a message like "Please upgrade your current CARS email account first, then try to request an additional email alias." if the current email is not jesed. It will use CARS_DBA.CARS_UTILS.NEWACCOUNT2 to get an alias.
- 3. On the Reset Password page:

Do not change the password for the jesed UCS account
- 4. On the Associate AD Account page:
